We are delighted with your interest in joining our Lincoln Square Synagogue community.  Help us best serve you by completing the following information:

Applicant Information:

Spouse Information (If Applicable):

Children's Information:

Check off if you would like to join our Chesed what’s app group. As part of the chat, you have the opportunity do small acts that can make a big difference. It is used only to alert community members to volunteering opportunities to help other people.
For example :
- A house bound neighbor needs some groceries picked up
- A shiva house needs a 10th man for a Minyan
- A meal train is being set up for someone home from the hospital
Joining the group does not mean that you will ever be personally, directly, asked to participate in any given mitzvah, or create an expectation or obligation. What it does is allow people who are in a position to help have the chance to do so.
